Limited reCapcha's ?

on my site i am implementing reCaptcha in my contact modals, however only one recaptcha is rendering (http://bit.ly/1F2ZaDP) as you can see on the contact us page it renders and works however on the service and review (http://bit.ly/1IRK6Ab) it dosent, please help ASAP with a reason and or fix -- You received this message because you are subscribed to the Google Groups "reCAPTCHA" group.
