Problem With Form After Putting In Captcha Code

I'm having a problem with my form code after I put the captcha code in. The Captcha code is working fine, but I cannot get it to pull up the correct page when the form is submitted correctly. Instead it just goes to a blank page - the form gets sent and comes through the email, but it doesn't notify anyone of that, so they have no way of knowing if it worked or not.
