Cannot get Captcha to show up - and what do I have to do with the php? I'm very confused

I hate to say this, but I cannot get capcha to show up on one of my client's web sites, and what do I have to do with the php?And it doesn't show up on my test server, which is my own server, http://bit.ly/1ryLtaW does not show up on her web site, nor does the page come up:http://bit.ly/1ryLtaY do I have to do to make all this work? What do I have to do with the php code for the captcha and the library code?
